Prayer Chain Confidentiality Agreement
Spirit of Joy Lutheran Church's Prayer CHain Covenant for Confidentiality.

This is to certify that as member of the prayer chain at Spirit of Joy Lutheran Church, I understand that any information (written, verbal or other form) obtained through the prayer chain must remain confidential. This includes all information about members, families or employees, as well as any other information otherwise marked or known to be confidential.

I understand that any unauthorized release or carelessness in the handling of this confidential information is considered a breach of the duty to maintain confidentiality.

I further understand that any breach of the duty to maintain confidentiality could be grounds for immediate removal from the prayer chain.

By signing below, I agree to participate in prayer and caring ministry for my fellow Spirit of Joy partners, their friends, families, and all of my neighbors in Christ with the utmost respect and dignity as befitting all children of God.
